"I hereby declare to Helix Sports Performance, its directors and coaches that I am physically capable of participating in activities prescribed by the coaches/associates and that there is no medical reasoning for myself and my person endangering my health during tasks and or activities that are given. I acknowledge that whilst under the instruction of Helix Sports Performance coaches and its associates I will not hold them reliable or responsible for any liability, personal injury and damage. I herby release and will indemnify and keep indemnified the Helix Sports Performance coaches, directors and associates for any injury, loss or damage suffered whilst under their instruction. "

 

CLIENT/ATHLETE AGREEMENT

Initial commitment: Your initial commitment will be a minimum of training 1 to 4 times per week for 12 or 24 weeks (or agreed upon program length). You can pay up front via EFT, bank deposit or cash. Alternatively weekly direct debits are run every Tuesday for the week ahead.

Session expiration: The minimum amount of sessions you will complete per month is 4 to 20 dependent on your package type. So that is based on you doing 1 to 5 sessions per week. If you are unable to commit to you’re allocated time slot/s or unable to find time to work in and re book in your scheduled sessions within the week you will not be credited for the missed sessions. Missed sessions will not be carried over into the next week. Once week is up you must have completed all of your paid sessions otherwise you lose them

Length of sessions and appointment times: Each training session is based on a 50-60 minute workout. To get the most out of our efforts, please be ready to exercise at the appointed time. If you are more than 15 minutes late for a scheduled session, it will be considered a no-show and you will be charged.

Cancellation policy: All sessions are arranged on a scheduled appointment basis. In order for effective use of time, all clients are asked to give a 24-hour notice when canceling an appointment. This means a cancellation should be made at least 24 hours before the scheduled appointment by calling us directly. (Monday appointments must be canceled by Friday.) Personal training sessions canceled inside of 24 hours of the scheduled appointment will be billed at the normal rate of a single session and deducted from your account.

Cancelling in advance: If 24 hours notice is given to reschedule your session we will endeavor to move you to a more suitable time frame within our working hours.

Expiration of sessions: If you fail to complete the scheduled sessions for the length of your contract, you understand your training sessions will expire and you are responsible to pay any remaining balance of your account.

Release of results upon program completion: Client photos and testimonials are requested upon initial program completion (12 weeks).

Cease of program: All programs can be stopped at anytime although this will incur a fee of 50% of the remaining amount due, this is non-negotiable and will be enforced legally.

Nutrition & supplementation: Helix Sports Performance does not provide any medical diagnosis or advice regarding an individual’s physical or medical condition. The nutritional information that we provide is given as a guideline only. It is the sole responsibility of the client/individual to provide complete and accurate information so that the appropriate nutritional guidelines can be given to them. By agreeing to this I acknowledge that all guidelines given to me by my coach at the company are guidelines only and that I legally am responsible for all food and supplementation that I consume. I hereby relinquish and waive all responsibility of any health complication that may arise by following the nutritional guidelines given to me by my coach at the company

The relationship between you (our client) and us (your coaches) is a partnership. And only through working together can the mutually beneficial goal (your results) be best achieved.

Assess and determine together realistic health and fitness goals and put them on a timeline for achievement

Design a complete program to achieve your goals that includes:

Nutrition and supplementation – to fuel your body with the energy required and maintain a muscle- building fat-burning state

Resistance training – to increase your strength and build lean muscle

Cardiovascular exercise – to optimize fat burning and heart health

Stretching – to accelerate your recovery from exercise and prevent injury

Coaching – to motivate, support, and encourage you when required.

Teach you all the components of your program and help you implement them into your daily habits and life

Meet with you every 4–6 weeks to assess your progress, and update your program accordingly so you continue

to achieve the best possible results from your investment with us.

Based on our commitment to you, we require the following from you:

Workout a minimum of 2–3 days per week following the Helix Sports Performance program for a minimum of 12 weeks (cancelled training sessions must be made up during the same week or the following week)

Complete cardiovascular activity exceeding twenty (20) minutes per day at least three times per week

Fill out your Accountability Food and Cardio Journal in its entirety each day

Follow your custom-designed nutrition plan.

After the twelve (12) week period, if you can honestly say you followed all four of the requirements and do not look or feel better than you did when you first stepped through our doors, we will refund your entire investment.

Payments

We offer 2 methods of payments.

1. Direct debit- Debits are taken for the week ahead and are debited on a Tuesday. Payments can be stopped, altered, changed or refunded at anytime. There is no risk and saves us ALOT of time. 2. Cash payment 1 month in advance- Sessions are to be calculated for the month ahead and paid in full for that time period (For ALL programs including: private training, semi private training, group training, ladies sessions, rehab)
